A miner was exiting the skid steer EE was operating when EE slipped and contacted a sharp angle on the manufacturer built fork attachment causing a laceration that needed stitches.

Performing maintenance on catwalk of a shaker/screen that was broken down. EE walked under a belt & must have struck the shaker. A 4-6in rock somehow shifted & came down, striking EE's hard hat, then bouncing onto EE's glasses & nose. We just got confirmation of a fracture.

While lowering the new housing tube into position, the employee's left index finger was smashed between the tube and the side sheet of the vibrating screen machine. Finger was lacerated but not fractured. six stitches.
Welding back side of hopper frame, EE was on one side of hopper, another EE on the other side of the hopper. There was frozen material build-up on catwalk of hopper and scrap steel in the area. EE slipped and fell to the ground.

Getting in/out of truck to load bins. Exiting truck, put foot down on what he thought was a step, but was fuel tank. As he attempted stepping to catwalk, his foot slipped, he fell down, hitting wrist on catwalk
Replacing a return roller. While repositioning the roller, it slipped off the pin, fell to the frame, striking finger. Right 4th finger distal phalanx fracture.

Using knife to cut a notch in belt to replace a belt scraper. He drew knife towards his body, knife slipped & cut him in upper right leg. Treated at Occ Health, sutures received and removed. No work missed.

Employee was moving rocks in the muck pile to find the tooth of the shovel bucket. Employee had a rock in his hand and another rock rolled down on his hand smashing his left hand between the two rocks. Employee received a contusion to his left hand. Employee is to wear a splint but is doing his regular job.
Employee filled the truck with fuel and was walking to fill out the fuel ticket and tripped over the fuel hose causing him to fall and as he fell he wrenched his left elbow and shoulder. He received a strain to the elbow and shoulder. He is restricted.
Employee was cutting a side belt for the conveyor. He was using a box cutter knife. The kinfe slipped and employee cut his left index finger. Six stitches were required. There is no lost or restricted time.

EMPLOYEE WAS WORKING ON THE BIN. THE BIN HAD BRIDGED UP AND EMPLOYEE WAS HITTING THE SIDE OF THE BIN WITH A HAMMER. EMPLOYEE HIT THE CROSS-BRACE OF THE BIN WITH THUMB & SMASHED HIS RIGHT THUMB WITH THE HAMMER HANDLE. SIX SUTURES WERE TAKEN & HE WAS RESTRICTED.
EE WAS TAKING LUGNUT OFF HAULUP TRUCK. EE WAS USING A WRENCH. THE EXTENSION FROM THE BREAKER BAR TO SOCKET BROKE AND A PIECE OF THE WRENCH FLEW UP AND HIT HIS MOUTH BREAKING 10 TEETH. FRACTURE TO LEFT JAW BONE AND CUTTING THE INSIDE OUTSIDEOF HIS MOUTH. HAD 50 STITCHES INSIDE OF MOUTH AND 8 ON OUTSIDE.
