US Aggregates Inc · Fall from machine

Loader operator was exiting loader. EE had three point contact on the hand rail. EE's right hand slide off the rail causing EE to loose balance. EE swung to the left and couldn't hold self and fell. EE landed on EE's right side.

A miner was working underneath the Jaw Crusher swinging a Sledge Hammer to knock out a piece of metal on May 27th when EE strained EE's lower back. The miner had previously reported EE's back was bothering EE on 5/11/2022 while working at the mine. At that time no restrictions were put in place and it was not reportable. The miner had previous unrelated back injuries.

Replacing broken steps on a staircase, outside, on a very hot day. Employee felt overheated. Ambulance had to be called, treated for heat exhaustion with fluids, released from hospital later that day.

Employee was going down to work on a shaker, slipped on wet chute slime and caught self with right arm. From the date of incident 5/5/21 to 7/21/21 employee worked normal duty and normal job. On 7/21/21 employee had out patient surgery to fix rotator cuff.

Turning of drive belts. Right middle finger pinched between motor pulley and drive belts. EE jerked EE's hand quickly which severed the tip of finger. No bone loss. Y flap to cover wound.

Per employee his hands have been bothering him for a while but has not got any medical treatment until 1-02-13. Employee is restricted, is still working.

Employee claims he had felt the pain several days before but did not report it until 9-30-06 because he thought the pain would go away. He sought medical treatment on 10-02-06 and he is restricted.

EE DIDN'T REPORT IT TO HIS SUPERVISOR UNTIL 3/6/06 AND THEN HE WAS SENT TO THE CLINIC. HE REC'D BILATERAL MEDIAN NEUROPATHY IN BOTH WRISTS. HE IS RESTRICTED.

Employee was at the secondary plant and was stock piling crushed stone from the plant to the yard. After loading his truck, employee was getting back in the truck when he slipped on dust and loose stone. Employee fell and twisted his left ankle. Employee received a left ankle sprain and is restricted.

Employee was removing the pivot joint bushing on the Cat. 769 Haul Truck and replacing the bit on the air hammer. Employee placed his hand on top of the bit to push it in place, the trigger on the air hammer was accidently pulled causing the tool to be activated, resulting in a contusion of his right hand. Employee was restricted for 5 days.

Employee and a co-woker were pushing screen deck into place as it cleared the back rail of the shaker. The screen deck dropped and pinched emloyee's left thumb between the back rail and screen deck. Employee received a laceration of his left thumb that required 3 stitches.

EMPLOYEE WAS PULLING ON A CONVEYOR BELT WITH ANOTHER EMPLOYEE. EMPLOYEE FELT A POP IN HIS NECK. HE THOUGHT HE WOULD BE OK. LATER HIS NECK BECAME STIFF & PAINFUL. EMPLOYEE WENT TO THE DOCTOR 10-20-03. HE WAS IMPROVING BUT STILL HAD SOME PROBLEMS. HE WENT TO THE CLINIC ON 11-10-03 AND WAS GIVEN PHYSICAL THERAPY TREATMENTS MAKING IT RECORDABLE.
