A miner got a piece of slag/metal stuck in left eye and it had to be removed by the eye doctor. 0n 4/8/2023. The miner was wearing a proper welding helmet and safety glasses while welding during the day on 4/7/2023.

Cleaning up materials on the decking around the new crusher platform. Employee had harness on, the lanyard caused employee to fall into the hopper opening, striking lower back against an I-beam.
