Employee was shoveling stone off conveyor belt when slipped and hit nose against I-beam.

Employee welded a nut to the head of a bolt that was worn down to be able to remove it. When they went to actually put pressure on the hand tool and welded nut, the nut broke off and caused their hand to slip and hit part of a crusher table.

While working on a conveyor employee picked up a wrench to hand to another employee. When the employee took a step and turned EE felt a pop in EE's right knee.

Miner was on top of a crusher waiting to unload a wear plate EE was going to install in the hopper above EE. While the plate was being set on top of the crusher for EE with the tele-handler, it shifted and the miner tried to grab it to keep it from falling. In doing this EE strained a muscle in EE's lower back.

A valve for the lid lift sys. on a ISC 77 crusher stuck in the open position letting the pressure off the hydraulics. This allowed the lid to descend. Employee didn't install the safety pin in the lift system to prevent this before doing maintenance. As they moved the lid over for inspection, it descended unnoticed and pinched EE's finger between the lid and a hold down loop.
Employee was stepping down two steps onto a landing that is built with expanded metal for the platform. There is a slight bend in the catwalk where it's welded to an I beam for support. As the employee stepped on the cat walk, EE stepped into the bent area, causing EE to roll ankle which was later determined to be a sprain.

Miner was helping remove a bucket from a Cat 232D. As they hit the locking pins loose, from the dirt and mud build up, the bucket broke loose and swung forwards and backwards striking left foot. The arms of the skid steer were up and the bucket rolled forward putting the cutting edge on the ground. the operator was in the cab at the time.

988K Loader Operator did not see service truck or ground laborer near truck as they were backing up and struck the laborer in the lower back with the rear bumper of loader. The loader operator pulled forward dumped a bucket of material then backed up and struck the service truck.
Miner was using a 4 1/2"" grinder to cut a piece of screen cloth. The grinder had had the guard removed. As EE was cutting the screen it cut through the wire and hit the next one causing it to kick back and hit EE's left hand. Cutting EE between the middle finger and index finger, parallel to EE's fingers.
EE was putting a piece of scrap metal from the crusher (45-037) into the steel scrap hopper. While throwing it into the hopper EE felt something in the left elbow area give and a burning sensation started on the inside of EE's arm.
