Struck by falling object

Miner was removing a skid steer bucket when it fell on miner's left ankle.

Caught in, under or between a moving and a stationary object

Released clamps holding hammer assembly on rock drill. Upon releasing the clamp the hammer assembly slid forward pinning miner's hand against the front of the drill.

Employee was in the process of placing a drill steel in the drill mast. EE had one end on the pick-up tail gate and the other end on top of a berm. When employee pulled the steel onto the mast the back end of the steel fell off the tail gate pinching the employee's left hand between the steel and a large rock laying on the the berm.

Employee was walking from the scale house to his vehicle. He slipped and fell causing a fracture to the fibula bone on his left leg. The weather conditions was clear 28 degrees with the sun out.

BRIGHT/SUNNY AFTERNOON, TEMP. 82 DEG. EE WAS ATTEMPTING TO GREASE MOTOR ON A 60""-WIDE CONVEYOR. EE PULLED HIMSELF UP APPROX. 2 FEET AND RAMMED HIS HEAD INTO THE MOTOR HOUSING, CAUSING HIM TO LOSE HIS GRIP AND FALL BACK. EE LANDED WITH FULL WEIGHT ON HIS RIGHT LEG, CAUSING SEVERE PAIN AND DISCOMFORT.

EMPLOYEE WASLINING UP BACK PLATE FOR PRIMARY CRUSHER WHEN THE PLATE SHIFTED. THE EDGE OF THE PLATE KNOCKED OFF HIS HARD HAT AND CAUSED A LACERATION TO THE LEFT EAR AND A SMALL LACERATION TO HI S FOREHEAD.
