Released clamps holding hammer assembly on rock drill. Upon releasing the clamp the hammer assembly slid forward pinning miner's hand against the front of the drill.

Employee was in the process of placing a drill steel in the drill mast. EE had one end on the pick-up tail gate and the other end on top of a berm. When employee pulled the steel onto the mast the back end of the steel fell off the tail gate pinching the employee's left hand between the steel and a large rock laying on the the berm.

EE PULLED HIMSELF UP APPROX. 2 FEET AND RAMMED HIS HEAD INTO THE MOTOR HOUSING, CAUSING HIM TO LOSE HIS GRIP AND FALL BACK. EE LANDED WITH FULL WEIGHT ON HIS RIGHT LEG, CAUSING SEVERE PAIN AND DISCOMFORT.

STEEL CHANGER ON ROCK DRILL WAS NOT WORKING PROPERLY. MAINT. PERSON & EMPLOYEE WERE WORKING ON PROBLEM WHEN STEEL RELEASED DROPPING ON EMPLOYEE'S RIGHT FOOT. EMPLOYEE HAD LACERATION AND FRACTU RE OF RIGHT FOOT.
