IE WAS HOLDING A HAMMER AGAINST A PIN. A COWORKER WAS USING SECOND HAMMER TO HIT THE FIRST HAMMER TRYING TO JOLT PIN FREE. TWO SMALL PIECES OF METAL BROKE OFF THE HAMMER IE WAS HOLDING AND PUNCTURED IE'S RIGHT HAND.

The miner hit a large safety pin on a dolly leg of a surge bin with a hammer. The miner felt something hit left eye as miner bent over to swing the hammer. The miner left the mine site near the end of the shift without telling supervisor miner was hurt.

While trouble shooting an issue with the primary crusher, the employee was rotating the impeller by hand when his glove caught between the impeller and the liner pinching the tip of his right middle finger.

While working the lime stockpile, a large portion of lime broke loose and hit the loader directly behind and to the left of the lift arms. The bent the bracket holding the outside mirror bring it through the door window. This caused the operator to sustain several superficial lacerations on his left arm.

While assisting in the performance of maintenance on a Cat 330 excavator, employee struck in the right jaw area of his face with a glancing blow of a 16 pound sledge hammer. He was holding a metal bar, "with the use of a tie line"; the hammer glanced off the bar and still managed to strike him in the right jaw area of his face.

ON THE 20TH OF JANUARY 2003 AT APPROXIMATELY 3:20 PM EE A LOADER OPERATOR FOR DOUDS STONE, INC WAS ATTEMPTING TO MOVE A 4" WATER LINE THAT WAS USED TO PUMP WATER FROM THE PIT TO A SETTLING POND, WEATHER CONDITIONS AT THE TIME OF THE INCIDENT WAS CLEAR AND COLD, +OR- 17 DEGREES AND THEWATER LINE WAS FROZEN IN THE MUD. WHILE PULLING ON THE WATER LINE HE FELT A TWINGE AND BURNING

EMP HAD PARKED THE DOZER ON AN INCLINE AND HAD GOTTEN OUT OT MOVE AN ELECTRICAL CABLE. WHEN HE CLIMBED BACK ONTO THE DOZER IT BEGAN TO ROLL. THE EMP JUMPED CLEAR OF THE DOZER LANDING IN A SHAL LOW PITCH. THE LANDING FRACTURED HIS RT ANKLE. THE DOZER ROLLED APPROX 120' AND STOPPED.
