Employee was positioning his mobile equipment for refueling. While climbing out and as he stepped onto the ground, he twisted his right ankle and went down. Equipment was parked on a level area and the surface area was flat and free of tripping hazards. He stated that he isn't sure just what happened.

HIS TAILGATE WOULD NOT CLOSE. A MINE EMPLOYEE OF OURS BROUGHT A LOADER TO HELP CLOSE THE TAILGATE. INJURED HELD A BOARD ON THE TAILGATE SO IT WOULDN'T GET SCRATCHED. WHEN THE LOADER MADE CO NTACT W/TAILGATE, INJURED'S FINGER WAS PINCHED. HE HAD A CUT THAT REQUIRED THREE STITCHES.
DUST PARTICLE IN LEFT EYE, EE WAS CHECKING LEFT FRONT TIRE & WHEEL WHEN GUST OF WIND BLEW SPECK OF DIRT IN HIS EYE. SAFETY GLASSES WERE BEING WORN AT THE TIME OF ACCIDENT.
EE OPERATING DOZER. HYDRAULIC LINE RUPTURED. FIR RESULTED CAUSING 10% 2ND DEGREE & 90% 1ST DEGREE BURN TO THE FACE, NECK AND HANDS. EE JUMPED FROM DOZER AND DISLOCATED THE RIGHT SHOULDER.
