Employee was using an utility knife to cut a hose off the radiator of a portable water pump. EE was cutting with right hand, they allowed their left hand to get in the way. The blade passed through the hose and made contact with left thumb. Employee was taken to clinic where they received 5 stitches. They returned to work after leaving the clinic. No restrictions, no lost time.

Accident type, without injuries

The affected building is a 9-bay retail rental storage unit with 7 private renters. 2 of the bays are used by the operator- one as a wash bay and the 2nd one is primarily for storage of supplies unrelated to mining. It was Sunday, no mining activities were being conducted. The only EE on-site was in their office. No injuries. Report is being submitted to have citation terminated.

Caught in, under or between a moving and a stationary object

While driving a brass rod out of a dredge chain link, EE used an 8 pound maul with two hands. EE slipped with right hand. As the momentum of EE's swing continued, EE's right index finger landed between the maul head and pin as they collided.

WAS LIFTING UP ANCHORS WITH A HAND WINCH AND EE THOUGHT THE WINCH WAS LOCKING AS IT WAS RAISED. WHEN I RELEASED THE HANDLE IT SPUN AROUND AND HIT EE'S RIGHT THUMB. UPON XRAY IT WAS DISCOVERED THE TIP OF EE'S RIGHT THUMB WAS BROKEN. A SPLINT WAS APPLIED.

MINER WAS WORKING ON A CONVEYOR APPROXIMATELY 15' OFF THE GROUND REMOVING A ROLLER NEAR A WRAP PULLEY. A CHAIN SUPPORTING THE WRAP PULLEY STRUCTURE SLIPPED AND THE STRUCTURE WITH THE MINER ON IT DROPPED SEVERAL FEET. THE LEFT ARM WAS ON THE CONVEYOR FRAME AND WHEN HE DROPPED HIS ARM WAS FORCED UPWARD. INJURIES INCLUDE STRAIN TO THE LEFT SHOULDER.
