Employee has demonstrated a change in their hearing sensitivity as outline by MSHA. The employee was tested on 2/20/19 and retested on 3/25/19. The tests indicate a moderate loss at some high frequencies in the right ear.

The employee tripped over a pallet and fell. Employee tore the Achilles tendon.

The employee was picking up a piece of stone when a sharp part of the stone punctured EE's finger where it meets the palm. The employee received first aid treatment and when EE returned to the task EE discovered could not bend finger. EE did not indicate was in any pain, however it was obvious something was wrong.

A fire started in the production plant near the wood stove. The damage that ocurred was to the roof, plumbing, multiple electric circuits, rail cranes and the building interior.

CHUNK OF GRANITE FELL OFF OF CONVEYOR ON TO EE'S FOOT.

WHILE MOVING A 500 POUND GRANITE BLOCK ALONG THE CONVEYOR, EMPLOYEE CUT HIS HAND AND IT REQUIRED 8 STITCHES.

WHILE HAMMERING A CHISEL, HE HIT A GLANCING BLOW, CAUSING A PIECE OF STEEL FROM THE CHISEL HEAD TO GO INTO HIS LEFT HAND.

WHILE LIFTING AND TURNING WITH A 75 POUND PIECE OF GRANITE BLOCK, HE PULLED A MUSCLE.

HE STRAINED HIP MUSCLES WHILE PUSHING GRANITE CURB ON CONVEYOR.
