Employee wasn't paying attention to where EE was going and walked into a guard with their face. EE has damage to the teeth and they're checking EE's head and shoulder for other injuries.

Wood got stuck in the tail pulley. Employee used a piece of pipe to hit it out. Swung pipe, Clipped their finger and tore the fingernail.

While sharpening drill bit the grinding tool disengaged and hit the operator in the ankle causing a contusion.

Employee was loosening the ball valve on the Deister Triple Deck Wash Plant. Employee was using a pipe wrench and felt a pain in this shoulder. Employee left work and consulted a Doctor.
Employee was driving a haul truck, hit a bump and hit their head on the roof. Employee said they were wearing the seatbelt. Driver had the seat adjusted too high, was possibly driving too fast and wasn't reading the road. Employee did not report the incident until 2/16/17 after a Dr's visit where they were given the diagnosis of cervical strain.

Worker walked into a safety screen while checking a problem with the portable plant. Worker was wearing a hard hat and safety glasses and still scraped upper cheekbone, nose and eye.
Employee was working on a stalled conveyor. There was a small amount of loose gravel on the belt and employee slipped and fell injuring back.

Slip and Fall due to extremely low temperatures and icy conditions in the yard. Sand had been put down to prevent this, but the sand and snow wore down and this icy patch unknowingly was exposed as workers traversed the area.

Rocks bridged up in the hopper and spilled over the edge. When EE went to clear the jam - he rolled his ankle on the spilt rocks while walking to the hopper.
