EMPLOYEE WAS PULLING ON A PRY BAR. THE BAR SLIPPED AND HE OFF OF THE WALKWAY ONTO THE TRACK AND THEN HE FELL TO THE GROUND.

Michigan Paving and Materials Company · Caught in, under or between a moving and a stationary object

Employee was removing spray bar clamps from an old wash screen. After removing the hardware, the U-bolt was stuck in position around the bar, Employee used a 10" pry bar to free the clamp. While pulling downward the pry bar slipped and pinched the palm side tip of EE's left ring finger, causing a laceration requiring two stitches.

Employee was helping a co-worker that was welding when employee received welding flash to eyes. Employee was not wearing a welding helmet. The employee went to the ER on 3/18/23. The employee didn't report the incident and the superintendent was made aware that the employee went to the emergency room when we received an inquiry from our workmen's comp agent on 4/5/23

Miner was inspecting primary crusher when EE's left hand made contact with a jagged edge on a lift leg. Miner cut left palm and received stitches.
