Employee was prying apart 2 pieces of plate to add a spacer to be able to cut the top piece. While prying the pry bar slipped out from in between the two pieces and struck the employee in the lower mouth causing the inside of his lip to be cut by his teeth.

The MSP contacted Stoneco on Monday, July 28, in regards to a missing Monroe resident. The individual was last seen in the Dunbar & South Telegraph area on March 15, 2014 at 11:15 am. MSP requested to search the Northeast high wall & slurry ponds for the missing individual. At 11:19 am on July 30th the MSP K-9 unit discovered human remains at the base of the Northeast high wall.

Employee and Co-Worker were working on a CAT 365 Excavator in preparation of moving the machine to another facility. The injured employee was struck by a ladder when the counterweight was removed. The injured employee was standing on the ground and fell when he was struck by the ladder.

THE EE'S ILLNESS WAS AN INFECTION TO HIS FINGER. HE RECEIVED TWO INJECTIONS FROM THE DR. & RTW THAT DAY.
