Michigan Paving and Materials Company · Over-exertion in pulling or pushing objects

Employee was removing a 5-6' diameter 3000 lb drive sheave on a horizontal impact crusher to prepare rotor for removal. Sheave was seized on shaft and popped off once it came loose.

Michigan Paving and Materials Company · Struck by falling object

Miner was removing a guard to do maint. The guard slipped & landed on his left foot. The miner completed his shift & worked his next 4 scheduled days without incident. When he woke up the morning of 7/22 his foot was swollen had been on his feet all day 7/21. He came into work 7/22 & ran the plant with his foot elevated & iced. Miner left early to go to urgent care & get x-ray.

CONTRACT EMPLOYEE SLIPPED ON SNOW-COVERED WALKWAY WHILE CARRYING A GUARD. THE GUARD THE EMPLOYEE WAS CARRYING CAME DOWN ON HIS HAND, PINCHING HAND BETWEEN GUARD AND WALKWAY. EE RECEIVED CUT TO FINGER AND THUMB.

The employee suffered a laceration to the back of his head. He was wearing all proper personal protective equipment at the time of the accident.
