EE was walking from the Shop to the ready line to get in their haul truck when they slipped on a section of ice approximately 18 inches wide by 24 inches long. Roadway and walkway was cleared but miner did not see this small section when walking. Miner was prescribed a muscle relaxer and returned to work.

A miner was attempting to unplug a bridged rock out of the the portable jaw crusher when the employees finger got caught between a rock and the metal plate. The miner was wearing gloves but the rock smashed EE's ring finger causing EE to have to receive stitches at the end of EE's finger.

EE was adjusting carriage after team members had issues with busting bags. EE lowered carriage manual adjuster twice, the 3rd time EE did this EE felt a strain in EE's left lower back.
Escorting a visitor to a rented portable eagle crusher to verify the serial #. EE was holding the gate open at the top of the stairs, the visitor grabbed the gate for balance. EEs finger was pinched in the equipment. Occ Health referred EE to the ER as EE's nailbed had to be removed.
EE was filling 50lb bags at MCC. EE noticed a bag was positioned wrong and went over to fix bag on conveyor. EE didn't shut conveyor off and went over to grab bag and reposition it. While doing so EE hit hand on conveyor #2 bracket and cut hand and received a small pinky bone fracture.

NALC conducted our annual audiograms on 11/6/18. I received notice on 11/20/18 that an employee had results of an STS. This employee had given 2 week notice, prior to the audiogram. EE last day worked was on 11/9/18. Since ee is no longer employed here MSHA informed that we would need to fill out this form but not need to go any further with anything else.

While performing maintenance on plant grizzly hopper the miner went to tighten the bolts underneath. Miner's fellow coworkers were working above miner and when they went to move the light and tools the hammer fell between the grizzly bars. The miner looked up placing arm across to shield face. The hammer hit miner's arm and bounced off, hitting miner in the mouth.
Working on replacing v belts, pulled groin muscle and developed a hernia after trying to lift the motor using a crowbar.

EE was climbing down the steps of the water truck. As he placed his left foot on the ground, he felt his knee pop. Supervisor saw EE limping. EE said he would be fine continuing with his job. Approx 30 min later the Supervisor observed limp was more noticeable so he took EE to the ER. Taken off work for 2 days so he could follow up with his DR. 9/13/15 knee popped back in place.

EE was changing the air filter on a 345B Excavator and the air filter canister cut his thumb while he was placing the new filter in the canister. 3 stitch laceration.
