THE OVERHEAD F EED TO THE TRAN8FORMERS HAD NOT BEEN DISCONNECTED. I,NAME CAME IN CONTACT WITH THE OVERHEAD FEED TO THE TRANSFORMER WHILE REPLACING FUSED CUT-OUTS AND WAS ELECTROCUTED.

Employee stepped up onto the switch engine steps and felt knee give out. Employee reported having a history of pain in the affected knee and thought the pain would resolve itself, so didn't report the injury until a week after the incident occurred.

Employee was performing plant maintenance activities, which included removing a loose piece of steel that had just been cut loose from the plant. When the employee tried to remove the loose piece of steel, it dropped suddenly and pinched employee's right hand.

The employee was tightening a bolt on a dredge pump when the wrench EE was using slipped, causing EE to hit their hand on the pump and / or was struck on the wrist by the wrench.
Employee was completing a pre-operation check of a skid loader when EE stepped off of the skid loader track into a hole and twisted EE's ankle.

Employee was injured while guiding a 20" diameter steel pipe elbow, which was being lowered by a crane, onto some floor mounted pipe stands on the dredge. As the pipe was lowered, it was not seated properly on the stands. The crane was signaled to stop, but the pipe shifted and pinched the employee's lower left leg between the elbow and another piece of equipment.

Employee was grinding steel on the underside of the dredge ladder. The employee was wearing safety glasses and a face shield. A foreign object made its way into the employee's right eye. The injury was not reported until two days after the incident. The foreign object was found to be imbedded and then removed by an eye doctor.
