Employee was performing plant maintenance activities, which included removing a loose piece of steel that had just been cut loose from the plant. When the employee tried to remove the loose piece of steel, it dropped suddenly and pinched employee's right hand.

The employee was tightening a bolt on a dredge pump when the wrench EE was using slipped, causing EE to hit their hand on the pump and / or was struck on the wrist by the wrench.

Employee was completing a pre-operation check of a skid loader when EE stepped off of the skid loader track into a hole and twisted EE's ankle.

Employee was using a cutting torch to cut a bolt on the bottom of a pipeline joint when the hot end of the bolt fell into employee's glove and burned hand.

Employee was injured while guiding a 20" diameter steel pipe elbow, which was being lowered by a crane, onto some floor mounted pipe stands on the dredge. As the pipe was lowered, it was not seated properly on the stands. The crane was signaled to stop, but the pipe shifted and pinched the employee's lower left leg between the elbow and another piece of equipment.

Employee was grinding steel on the underside of the dredge ladder. The employee was wearing safety glasses and a face shield. A foreign object made its way into the employee's right eye. The injury was not reported until two days after the incident. The foreign object was found to be imbedded and then removed by an eye doctor.

A 1 1/2 TON CHAIN COME-ALONG MALFUNCTIONED WHEN SWITCHING IT TO DOWN POSITION WHILE LOWERING IMPELLER ON DREDGE. FINGERTIPS WERE CAUGHT BETWEEN SUPPORT & POLE CAUSING BROKEN TIPS ON RING & MID DLE FINGERS OF RIGHT HAND.

PLACING BEAM UNDER THE DREDGE LADDER USING A CABLE COMEALONG. BEAM BECAME STUCK. HE PULLED UP ON THE CABLE TO FREE UP BEAM. INJURED LOWER BACK. 4TH & 5TH DISC IN BACK STRAINED.

EMPLOYEE WAS CROSSING BETWEEN 2 RAILROAD CARS, HIS FOOT SLIPPED, HE TRIED TO CATCH HIMSELF WITH HIS LEFT ARM AND PULLED A MUSCLE HIS SHOULDER.

EMPL DID NOT FOLLOW CORPORATE SAFETY POLICIES & ENTERED INTO HOPPER WHERE MATERIAL IS DUMPED. MATERIAL LOGED & WHEN IT BROKE FREE MATRIAL CONE FORCED EMPL THROUGH HOPPER OPENING ONTO CONVEYOR BELT & ONTO REJECT STOCKPILE.

INJURED PARTY W5S SERVICING DREDGE HE WAS LEANING OVER TO GREASE FITTING WHEN HIS FOOT SLIPPED & HE STRUCK HIS RT.LEG JUST BELOW THE KNEE

EMPLOYEE WAS GR9ASING THE HOIST PULLEY ON THE DIGGER CHAIN LADDER WHEN HIS FOOT SLIPPED CAUSING HIM T O TWIST AND AGGRAVATE AND EXISTING BACK CONDITION.

STEPPING OUT OF3ROWBOAT SLIPPED ON BANK AND FELL ON LEFT SHOULDER.BRUISED SHOULDER & SUFFERED SUBLUXATION OF 6TH CERVICAL BERTEGBRA PER CHIRPRATOR.

STEPPED ON SCRE9N CAUSING HIM TO FALL-ABOUT 14 TO 2 HOURS LATER HE FOUND A PRE-EXISTING HERNIA HASBECOME STRAIN LATER.DURNING SURGICAL REPAIR INTERNAL BLEEDING WAS FOUND.

PRYING ON BEARING WITH PRYBAR WHEN BAR SLIPPED AND STRUCK HIM ON FOREHEAD 3 SUTURES TO CLOSE WOUND

EMPLOYEE WAS STEADYING PONTOON SO PIPELINE SECTION COULD BE SET ON IT HIS LEFT THUMB WAS CAUGHT BETWEEN PIPE & ANGLE IRON BRACKET
