METSO PORTABLE 6007Metal/Nonmetal

Employee was working on crusher and struck (L) hand on the fan blade. Suffered (L) hand laceration, (L) fourth digit tendon laceration.

Employee was trying to free a hose that was frozen to the ground. When the hose gave loose, the jerk was sufficient enough for the employee's hard hat to fall out, allowing the hose to strike him in the head, causing a laceration.

EMPLOYEE WAS HAMMERING A STEEL WEDGE WHILE CHANGING DIES ON THE CRUSHER, A PIECE OF THE STEEL WEDGE BROKE OFF AND STRUCK EMPLOYEE IN THE LEG. THIS RESULTED IN A FOREIGN BODY LEFT LOWER LEG INJURY.

EMPLOYEE WAS CHOPPING ICE FROM THE PORTABLE CRUSHER TRACKS AND STRAINED HIS SHOULDER. EMPLOYED FAILED TO NOTIFY THE OFFICE OF HIS INJURY IMMEDIATELY.

EMPLOYEE WAS PRYING A ROCK WITH A CROWBAR. THE CROWBAR SLIPPED AND DROVE HIS KNUCKLE INTO CROSS BRACE ON CRUSHER.
