Employee was unhooking a generator trailer and using a tool to release the fifth wheel plate. Tool slipped off fifth wheel plate and employee feel backwards on to the ground hitting their head.

Pinched left hand index finger requiring stitches. Quarry worker was removing a pin from a hydraulic cylinder when the cylinder moved and pinched the finger between the pin and the sleeve on the cylinder.

The miner was working on a walkway greasing the plant and stepped backwards into a small opening next to the motor and lost his balance. He fell directly onto the motor which had a eye bolt sticking out of it. The miner fell only backwards landing on the motor and eye bolt.
