Employee was fueling an excavator, employee slipped off of excavator track, striking ground injuring left shoulder.

SIDWELL MATERIALS INC · Caught in, under or between running or meshing objects

Employee placed Left Middle and Ring fingers in a pinch point area while Shields were being lowered causing injury to employee's fingers, employee could not explain why employee placed them in a pinch point area.
