THE MAN STATED HE STEPPED DOWN FROM THE TAILPIECE. HE INADVERTENTLY STEPPED ONTO A SMALL ROCK ON THE MINE FLOOR. HE SLIPPED AND THIS CAUSED HIM TO TURN HIS LEFT ANKLE.
ESE USED RAIL BARS WERE BEING STACKED IN ROWS ON TOP OF TIMBERS AND WOOD THAT WERE PLACED ON OR NEAR THE GROUND. AS I.NAME WAS REMOVING THE CHODKER ROPE FROM AROUND THE HOOK ON THE END LOADER ONE OF THE RAIL BARS FELL FROM THE STACK AND STRUCK HIS RIGHT FOOT.

THE MAN STATES THAT HE WAS WALKING BACK TO THE FEEDER. HIS NACK SUDDENLY GAVE OUT. THIS 4-30-85 OCCURRENCE IS RELATED TO A 2-9-80 INJURY. PLEASE NOTE THE ATTACHED LETTER & DOCOTRS STATEMENT.

I.NAME HAD HIS LEFT F OOT ON THE CONVEYOR CHAIN. WHEN THE CONVEYOR CHAIN STARTED THE MAN\S FOOT WAS CAUGHT CAUSING INJURY TO LEFT FOOT & LEFT LEG.
