Employee was removing a cutting edge bolt on a loader bucket and it cut a finger. Was wearing mechanics vinyl gloves.

Employee was palletizing and used their left knee to push on the stack of rocks to straighten them causing a bruise reported on 8/9/19. Employee requested medical attention on 8/12/19.
Employee was palletizing landscaping rocks when EE felt a burning sting in the lower right side of the abdomen. The employee had hernia surgery early this year in February. EE was seen in urgent care at the Health Center that same day and had an ultrasound on 7/01/2019. Diagnosis was a strain in the lower right abdomen, no damage to the repaired hernia.

Employee was lifting roll of fencing to wrap pallets and as they sat down the roll they dropped the roll in a weird way and felt their shoulder pop.
Employee was using a pry-bar to dislodge a rock and their hand slipped and hit a piece of metal which sliced the right hand pinky finger.

Employee was pulling a rock from the sorting line to the discharge line and got finger caught between the rock and cross brace smashing the left ring finger.
Employee was palletizing rocks when ee pulled a stone from pile and another shifted and smashed middle finger on right hand.
Employee was processing stone and went to lift a piece of stone and felt a pain in their lower back. They refused medical at time of event. Employee went to doctor on 5/8/2017 and doctor put them on light duty.
Employee was palletizing stone and as ee stood up after picking up a stone ee felt pain in lower back. Employee did not request medical attention for this until 4/17/17.
Employee was walking along and suddenly had a seizure. This was a pre-existing non disclosed issue. Doctor said there was no injury from the seizure and no secondary injury from falling. No equipment was running at this time.
Employee reported on 3/21/2017 that they pulled lower back muscle when employee twisted while moving product

Employee was palletizing and as EE removed a stone from the pile to place on the pallet, another stone slid down and caught the employees right hand ring finger, crushing it.
