Tunnel feeder had concrete chunks get stuck on crossbeams and needed to be removed before any more material is fed into the feeder. Employee took a 12lbs sledgehammer and tried to break material, this did not work. After this task the Employee felt pain in their right hand. Employee did not report the pain until 03/27/2026 at 630am after medical treatment

ON 8/16/18, MINER REPORTED EXPERIENCING SEVERE BACK PAIN AFTER SITTING IN A CHAIR WHILE OPERATING THE DREDGE ON 8/14/18, FOLLOWED BY A 12 HR SHIFT PERFORMING MAINTENANCE ON 8/15/18. MINER INITIALLY REPORTED TO SUPERVISOR AND A GROUP OF MORE THAN FIVE OTHER MINERS THAT BACK PAIN WAS NOT RELATED TO WORK CONDITIONS.

Employee was replacing a 10" diameter pipe that went through the catwalk (expanded metal) that he was working on. After he removed the pipe and co-workers were raising the new pipe, the employee turned to retrieve an air hose and his left leg fell into the 10" hole in the catwalk where the old pipe used to be.

THE CARRAGE WAS SITTING IN THE UPRIGHT POSITION BEING HELD THERE BY THE BOUND CABLE. ATTEMPTED TO FREE W/HAMMER, CABLE CAME FREE, CARRAGE ROLLLED OVER PINNING EMPLOYEE'S ANKLE.

HE LANDED ON HI S NECK AND SHOULDERS. HE HAS 3 BROKEN VERTIBRAE IN NECK AREA. HE IS IN THE HOSPITAL IN SALEM OR DR DO NOT EXPECT PERMANENT PARALYSIS.
