EE opened an air line to bleed the system and forgot to turn off a ball valve at the air tank causing the air line that runs under the building to come up and out of the hole and hit EE in the head.

EE hurt EE's wrist shoveling under a conveyor.

Injured middle finger on left hand when EE was putting heavy chains away.

EE was assembling the bearing house on the 1B Wash Screw & smashed a finger

Inspecting conveyor belt when EE tripped over a rock and fell.

Was helping secondary plant operator clear jammed chute. Operator was putting rocks on belt and I was throwing them off. I was grabbing a rock and the operator did not see me when they dropped another rock and it landed on my hand.

EE was looking for hole to direct backhoe to dig for tunnel hole and fell into the hole.
