EE jammed EE's finger while slamming the e-stop in the drill rig after EE had hydraulic hoses burst. EE also caught it in the metal loop on EE's wheel chock within a 20 minute time span. EE's thumb started bothering EE later in the afternoon

EE was swinging a pick and hitting a frozen chunk of stones and mud, trying to breakup, and remove it from the catwalk. When EE hit the frozen pile, EE felt something snap/pop, and a sharp pain in right wrist.

EE felt a pop in EE's knee while getting up into the drill carrier cab.

Employee slicing cardboard borehole casing when knife slipped and cut left wrist.

Knocking rocks out of the holes in the top deck of the primary screen when a piece of rock hit mouth.

Fell over I-beam and landed on elbow

On 3 separate occasions, EE slipped on ice and fell.

EE hit hand with a hammer.

Wiping dust off windows of Loader, slipped while going back into cab. Foot slipped off the step at the cab and fell to the ground.

Left foot slipped off ladder, tried to get a better grip and fell backwards causing employee to bruise the right elbow

EE tripped on cat walk and hit right hand on something.

Employee was lifting material out of drill and strained wrist.

Replacing belts

Attempted to climb up on portable screen deck and missed footing and fell down on left thumb. Unable to use or move thumb.

Myself and co-worker were lifting a piece of sheet steel when the steel shifted away from us resulting in co-worker losing grip and it falling approximately 2 1/2 feet onto my left foot. Resulting damage is a hairline fracture to 1st metatarsal.

Employee alleges back pain when working on drill. Employee did not report to employer till June 10, 2011

STEPPED DOWN OFF OF LADDER OF 45 CRUSHER THE WRONG WAY AND TWISTED HIS RIGHT ANKLE.

BELT STOP AND HAD TO HOVE BELT OFF OF STONE

CLIMBED UP ON STONE SCREEN TO REANGLE CHUTE PLATE TO MAKE MORE STONE GO UP THE 2B WASH SCREEN. WHEN I FINISHED I TURNED TO CLIMB DOWN, MY KNEE POPPED GAVE OUT AND I FELL TO THE GROUND.

EMPLOYEE WAS DOING ROUTINE MAINTENANCE ON GRIT BELT. WHEN WORK WAS COMPLETED, EMPLOYEE WAS GETTING OF BELT AND HIS FOOT SLIPPED ON ICE. HE FELL APPROX. 4 FEET LANDING ON HIS BACK.

DOING GENERAL CLEAN-UP OF AREA, BENT OVER AND PICKED UP SMALL PIECE OF STEEL, TURNED TO LAY ON SCRAP PILE, TWISTED LOWER BACK.
