EE had just finished a repair to the surge. When coming out of the chute, some small rock slid off the shelf and slid across EE's head. This left EE with a cut.

EE went to slide a pad under the left of the crane, EE felt something pull in the right arm.

Employee claims while adjusting water flow at the secondary plant, they turned to move to another location and tripped. As a result, they hurt their wrist.

Repairing a water line when a bar dropped on finger

Doing pre-shift inspection of primary plant, tightening cheek plate bolt with a wrench when it slipped and hit the left front tooth causing it to crack.

Cutting a grease line when razor blade slipped and cut right thigh

Was driving a rock truck and it tipped backwards. Employee feels sore.

EE was reaching into work truck for a tool and when stepping onto truck step rolled right ankle.

Slipped on ice and fell, landed on back.

Unjamming Syntron and ripped off nail and skin almost to bone.

Skiving top layer of conveyor belt for splice. Pushing hot knife through and it slipped causing the blade to cut through the glove and cut left thumb.

Continuous operation of rock truck has resulted in low back and neck pain; Did not seek any medical attention until 05/12/11 and is diagnosed with cervical/trapezius strain/sprain.

Employee walking under belt with hard hat on and struck head against belt pushing hard hat into neck, resulting in neck strain.

BURNING STEEL, RUST POPPED UNDER SAFETY GLASSES.

EE CLAIMS THAT HE WAS BACKING DOWN OFF STOCKPILE HIT HOLE IN QUARTRY FLOOR CAUSING LOADER TO BOUNCE.

WHILE ATTEMPTING TO SLIDE DRILL HAMMER INTO POSITION, EMPLOYEE INJURED BACK.

EMPLOYEE LOADED 10"-12" ROCK ONTO DUMP TRUCK. TRUCK WENT ONTO SCALE DISCOVERED HE WAS OVERWEIGHT. WENT BACK TO SOCKPILE TO DUMP SOME ROCK OFF. EE WAS STANDING AT REAR OF TRUCK DIRECTING DRI VE AS TO HOW MUCH MATERIAL TO DUMP OFF. ROCK BEGAN TO SPILL OUT ONE ROCK APPROX 8"X8" CAME OUT AND FLEW BACK HITTING EMPLOYEE IN RIGHT FOOT HITTING AT BOOT LACES AREA.

EE WAS DRILLING BLAST HOLES. LOOSE ROCK COLLAPSED INTO HOLE. EE WAS CLEANING HOLE OUT BY HAND AND A ROCK FELL ONTO HIS RIGHT HAND.

EMPLOYEE WAS MOVING A RUBBER GUARD FROM BEHIND A PULLEY ASSEMBLY ONTHE PORTABLE CRUSHING PLANT THE RUBBER GUARD CAUGHT THE PULLEY & DREW EMPLOEES ARM INTO GUARD SCREEN

EMPLOYEE WAS CLEANING UP SCRAP STEEL AROUND OUTSIDE OF SHOP. CUT HIS LEFT HAND BETWEEN A PIECE OF SCRAP STEEL AND BUCKET OF LOADER HE WAS PUTTING THE STEEL IN.

EMPLOYEE WAS CARRYING PROPANE TORCHES UP TO THE TUNNEL TO PERFORM ROUTINE MAINTENANCE. STEPPED ON A ROCK, SLIPPED AND LANDED ON THE ROCK WITH HIS SHINE. WENT TO THE HOSPITAL TO GET IT CHECKED OUT. NO BROKEN BONES. NO STITCHES, BADLY BRUISED

OPERATOR RAN LOADER UNTIL 130 P.M. PARKED LOADER AND WAS TOLD TO HELP ON MAINTENANCE. OPERATORS WAS TOLD BY EE TO REMOVE SCRAP STEEL FROM MAGNET. MAGNET IS LOCATED ABOVE BELT AT PRIMARY CRUSH ER. CRUSHER WAS NOT RUNNING BECAUSE OF SCHULDED MAINTENANCE THAT DAY EE WALKED OUT ALONG BELT ON WIRE RACK APPROX. 12 INCH WIDE TO MAGNET HE WAS PULLING ON A PIECE OF STEEL PLATE ABOUT 3/8 X
