WE BELIEVE THE VICTIM WAS IN THE PROCESS OF ASSEMBLING A DETASLIDE DETONATOR INTO THE CAVITY OF A HDP-1 BOOSTER, WHEN THE CAP OF THE DETASLIDE UNIT SAW OUTSIDE PRESSURES TO THE EXTENT THAT THE CAP DETONATED & IN TURN DETONATED THE HDP-1 BOOSTER INSTANTLY KILLING THE VICTIM.

While performing maintenance on the El Jay crusher, a wrench (approximately 50 lbs) slipped and fell onto miner's right pinkie finger. Bleeding and immediate swelling occurred. Went to Emergency room which showed no breaks but a follow up with a hand specialist is needed to check for ligament or tendon damage.
