Another company came to tighten a bolt on the quick coupler of a CX350. They tried with a swivel head ratchet on the nut end & an air impact on the head of the bolt. It didn't work. Then switched places & our EE was holding the ratchet with a hand on the head of the bolt & tried to loosen it, the pivot pin in the ratchet broke & smashed EE's left thumb.

While putting a 30' piece of channel frame conveyor into the shop to avoid the rain the straps on one end slid off the forks on the telehandler allowing the end of the conveyor to fall aprox 3' onto an employees left foot.
