Shop employee was attempting to remove a trailer tire from the front axel of a tanker truck with a sledge hammer. The sledge hammer bounced back and pinched the employees hand against the fender of the truck causing a 1/2" laceration that required 2 stiches.

Employee was changing engine oil in a triaxle. EE was working in the lube pit when the accident occurred. A drop of oil fell between the employee's safety glasses and eye. Hence, employee got a drop of oil in EE's eye.

EE was entering the operators cab of a CAT 982M wheel loader. As ee went from the door way of the cab to positioning body in front of the operators seat, ee felt a pop in lower back. Then when ee sat down, ee had severe pain in right hip (sciatic nerve pain).

EE was descending the Water Truck with both hands free. EE stopped on the bumper step with both feet to relocate lunch box to the front bumper with left arm while maintaining 3 points of contact. EE slipped / fell 32" to the ground landing on both feet in the catchers position. EE fell forward hitting right shoulder on the lower step support bracket.

EE was recharging the Cooltech HVAC machine. EE closed the R134A refrigerant tank valve and then proceeded to remove the line set. There was a refrigerant discharge when the line set was removed because the R134A refrigerant tank valve was not completely closed all the way. The EE received a chemical burn on their left hand as they attempted to reconnect the line set.

EE was climbing the right rear steps of 992K loader. Their right foot slipped off 3rd step as their left foot was reaching for 4th step. EE fell 11 inches to the second step which they caught with their right foot. This incident resulted in a strain in the EE right hamstring. On April 26, 2016 Doctor prescribed Physical Therapy. Up until now, this has been First Aid case.

EE was attempting to remove a pin out of a snow plow assembly on a triaxle with a 5 lb hammer and punch. EE struck the punch twice and the pin did not move. On the third attempt, EE swung the hammer with more force and missed the punch and the hammer struck the top of EE's left thumb. Left thumb is swollen, but fortunately no broken bones.

While attempting to unsecured a de-engerized electrical cord that was held by zip-ties, employee cut towards himself with the knife feature of the Leatherman that he was using. After the zip-tie was cut, the momentum of the knife took the blade accidentally into employees's left forearm. The laceration required 7 stitches.

EE was checking tire pressures on Tri axel # 7-124. EE was spinning the lift axel tire to locate the valve stem to check the tire pressure. The lift axel was in the up position. During the spinning process, EE's hand became pinched between the tire and the metal fender. The metal fender cut the back side of his right hand at the base of the thumb.

EE was in the process of removing a motor base bolt. The 24" crescent wrench slipped off the nut and struck EE's right hand. EE went to receive medical attention due to the amount of pain he was in. Luckily there was not broken bones or stitches required. The Doctor put EE on restricted duty; no lifting items greater than 5 pounds.

Alex was in the process of loading a new screen cloth from the storage rack into a utility truck. The schroud (crimped edge of the screen) had a razor like edge caused by a bad shear in the manufacturing process. Alex cut his left thumb on the razor like schroud which resulted in 5 stiches.

Employee was standing near his handle that operates the truck bed tarping system. The tarp was jammed. A mechanic from the shop helped free the tarp system and pushed it with his hands. The handle came around and struck employee in the lower lip.

THE LOADER OPERATOR STARTED TO DUMP HIS BUCKET LOAD AND A LARGE ROCK ROLLED OUT OF THE BUCKET AND HIT THE TOP TIE HE WAS STANDING ON. HE FELL BETWEEN THE WALL AND THE EMBAN

UPON ENGAGING THEHYDRAULIC BED LIFT, THE DISPLACEMEN7 OF THE WEIGHT WAS ALTERED, THE FILL UNDER THE TRUCKS LEFT REAR WHEEL FAILED. DUE TO THE GRADE & POSITION OF THE TRUCK, IT FELL FROM LEFT TO RIGHT & FROM REAR TO FRONT.
