Employee was changing engine oil in a triaxle. EE was working in the lube pit when the accident occurred. A drop of oil fell between the employee's safety glasses and eye. Hence, employee got a drop of oil in EE's eye.

After removing a tire from the lift axel of a triaxel on GOH equipment number 7-194, the employee felt a pain in lower left back as EE stood back up.

EE was entering the operators cab of a CAT 982M wheel loader. As ee went from the door way of the cab to positioning body in front of the operators seat, ee felt a pop in lower back. Then when ee sat down, ee had severe pain in right hip (sciatic nerve pain).

EE was exiting the operator's cab of a tri axel facing forward and missing the first step and fell to the shop floor striking the torch cart with EE's right hand as EE fell.

Some paper fell behind a filing cabinet drawer. While employee was retrieving the papers, they placed their left hand on the cabinet drawer slide. A piece of sharp metal lacerated the left ring finger.

EE was descending the Water Truck with both hands free. EE stopped on the bumper step with both feet to relocate lunch box to the front bumper with left arm while maintaining 3 points of contact. EE slipped / fell 32" to the ground landing on both feet in the catchers position. EE fell forward hitting right shoulder on the lower step support bracket.

EE was recharging the Cooltech HVAC machine. EE closed the R134A refrigerant tank valve and then proceeded to remove the line set. There was a refrigerant discharge when the line set was removed because the R134A refrigerant tank valve was not completely closed all the way. The EE received a chemical burn on their left hand as they attempted to reconnect the line set.

EE was climbing the right rear steps of 992K loader. Their right foot slipped off 3rd step as their left foot was reaching for 4th step. EE fell 11 inches to the second step which they caught with their right foot. This incident resulted in a strain in the EE right hamstring. On April 26, 2016 Doctor prescribed Physical Therapy. Up until now, this has been First Aid case.

EE was attempting to remove a pin out of a snow plow assembly on a triaxle with a 5 lb hammer and punch. EE struck the punch twice and the pin did not move. On the third attempt, EE swung the hammer with more force and missed the punch and the hammer struck the top of EE's left thumb. Left thumb is swollen, but fortunately no broken bones.

While attempting to unsecured a de-engerized electrical cord that was held by zip-ties, employee cut towards himself with the knife feature of the Leatherman that he was using. After the zip-tie was cut, the momentum of the knife took the blade accidentally into employees's left forearm. The laceration required 7 stitches.

EE was checking tire pressures on Tri axel # 7-124. EE was spinning the lift axel tire to locate the valve stem to check the tire pressure. The lift axel was in the up position. During the spinning process, EE's hand became pinched between the tire and the metal fender. The metal fender cut the back side of his right hand at the base of the thumb.

EE was in the process of removing a motor base bolt. The 24" crescent wrench slipped off the nut and struck EE's right hand. EE went to receive medical attention due to the amount of pain he was in. Luckily there was not broken bones or stitches required. The Doctor put EE on restricted duty; no lifting items greater than 5 pounds.

Alex was in the process of loading a new screen cloth from the storage rack into a utility truck. The schroud (crimped edge of the screen) had a razor like edge caused by a bad shear in the manufacturing process. Alex cut his left thumb on the razor like schroud which resulted in 5 stiches.

Employee was standing near his handle that operates the truck bed tarping system. The tarp was jammed. A mechanic from the shop helped free the tarp system and pushed it with his hands. The handle came around and struck employee in the lower lip.

Employee was replacing a broken U-bolt on the rear suspension of a Triaxel. As he was hammering on the broken U-bolt after using a torch, a "bb" size piece of slag struck his left wrist and became embedded in his arm.

Injury was lifting a 5 gallon jug of antifreeze out of the bed of his pick up truck to restock the lubrication cabinet in the quarry. As He lifted the jug with his right arm over the side of his truck bed, he felt a sharp pain in his right shoulder and right elbow.

Empployee was rolling up a pressure washer hose back onto the hanger in the shed behind the Shop when he felt a pop in his lower back. He then had pain in his lower back and right testical.

Slip, trip and fall due to snowy and icy conditions; when she fell she injured her right knee.

Results of an audiogram showed that employee had hearing loss determined to be reportable.

Attempiting to clean return roller when hand slipped and left ring finger was caught between return roller and conveyor belt.

Employee was cleaning around tail pulley of conveyor. He tripped on a rock, catching himself with his left hand and injuring his left ring finger.

EE was cleaning oil from the bottom of a tank. Cut right forearm on sharp edges on side of tank.

EMPLOYEE NOTICED A SLIGHT PAIN IN HIS GROIN AREA AND POSSIBLY A SMALL LUMP IN THIS AREA ALSO. HE WAS DOING NON-PHYSICAL WORK AT THE TIME. (HERNIA WAS SURGICALLY REPAIRED.)

miner was out walking on piles trying to find a tooth that fell off the 992 loader when he stepted on a stone that flipped and twisted his ankle

employee was standing cleaning out a chute with a bar. as he brought the bar down with some force to remove material he pinched his finger between the bar and the edge of the chute cutting the the end of his left little finger about three quarters of an inch.

EMPLOYEE WAS HELPING TO CHANGE A BUCKET ON A 330EXCAVATOR HE STEPPED OVER TO HELP THE OPERATOR PUT THE PIN IN AND IT FELL OUT OF OPERATOR HANDS AND THE BUCKET THEN LANDED ON HIS FOOT. DR TOOK X-RAY AND SAID THE BONE THAT WORKS THE 4TH TOE ON RIGHT FOOT IS CRACKED.

EE WAS CLEANING TRACK ON CAT 376 HOE & STRAINED LEFT CHEST MUSCLE.

EMPLOYEE GOT CARPAL TUNNEL SYNDROME IN HIS LEFT WRIST.

TRYING TO REMOVE A ROCK THAT WAS STUCK ON CONVEYOR. CONVEYOR WAS STOPPED, THE ROCK FELL ON EMP RIGHT THUMB THAT WAS PUSHED AGAINST A PIECE OF PERMANENT ANGLE IRON. DR STITCHED THUMB UP AND EMP WILL HAVE FULL USE WHEN HEALED.

CUTTING HOSE WITH A UTILITY KNIFE, WHEN EMPLOYEEGOT TO END OF HOSE, THE KNIFE SLIPPED, AND CUT HIS LEFT HAND.

EE WAS LIFTING A ROCK, THE ROCK SLIPPED & FELL ON HIS HAND CUTTING HIS MIDDLE FINGER

WHEN PLACING FORKS ON TOWMOTOR, FORK SLIPPED OFF OF MACHINE, INJURING THIRD AND FOURTH FINGERS ON RIGHT HAND.

EE WAS HELPING TO LOAD SOME SCREEN ON THE BACK OF A PICK UP TK. THE RIGHT INDEX FINGER GOT CAUGHT AND WAS CUT.

THIS IS A RECURRENCE OF A PREVIOUS BACK INJURY THE RECURRENCE HAPPEN WHILE HE WAS SHOVELING AROUND THE CRUSHER AREA.

THE EE WAS RETORQUING HEAD BOLTS ON TRUCK WHEN HE GRAVOUNTED HIS LUMBAR SPINE.

THIS EE STARTED SEEING A CHIROPRACTOR ON 8-16-93 FOR HIP PAIN HE HAS GAVE FOR THE LAST 6 WEEKS 3 TIMES PER WEEK ONT HE RECOMENDATION FO THE CIROPRACTOR. THERE ARE O ACCIDENT OR INCIDENT REPROT S DUE TO THE EE STATION HIS HIP PAIN WAS NOT WORK RELATED. THE CHIROPRACTOR THEN SUGGEST THAT HIS HIP PAIN WAS POSSIBLE WORK RELATED DUE TO YEARS OF EQUIPMENT OPERATION. AGAIN THE EE SAID HIS

EMPLOYEE WAS WALKING OVER AREA HE WAS WORKING AND SLIPPED ON A ROCK TWISTING HIS LEFT KNEE.

EE WAS DECENDING ON LADDER WHEN FOOT SLIPPED IN THE PROCESS EE PULLED MUSCLE IN LOWER BACK CONDITIONS OF WORK AREA WERE WET AND SLIPPERY.

EMPLOYEE WAS SHOVELING MATERIAL FROM UNDER A CONVEYOR WHEN HE STRAINED HIS LOWER BACK IN THE PROCESS

THE EMPLOYEE AS OPERATING A D10N DOZER PUSHING AMUCK PILE DOWN TO A LOADER. WHEN HE WAS HALF THE WAY DOWN THE MUCK PILE HE HAD SEVERE LOWER BACK PAIN. TALKING TO THE OPERATOR HE SAID HE DID NTO HIT ANY LARGER OR SOLID MATERIAL WITH THE DOZER CAUSIGN ANY SUDDEN STOPS OR SHIFT OF HTE MACHNE. HIS LOWER BACK PAIN SEEM'S TO BE AN IRRITATION OF A PREVIOUS STRAIN. EMPLOYEE HAD MEDICAL

EMPLOYEE WAS REMOVING A TRANSMISSION HE TWISTED HIS BACK IRRITATING A PREVIOUS BACK INJURY.

EMPLOYEE WAS LIFTING RADIATOR OUT OF MACK TRIAXLE TRUCK PULLED LOWER BACK.

I.NAME WAS HELPING SHOP PEOPLE PUT GUARDS BACK ON 988 LOADER. HE FELT A SHARP PAIN IN HIS BACK AFTER THE WORK WAS DONE, HE IS NOT SURE WHAT EXACTLY WHAT CAUSED THE PAIN. THE DOCTOR DIAGNOSIS W AS THAT HE HAD A PULLED MUSCLE.

EMPL WAS WORKING ON 25 TON CRANE TAKING OFF SNAP RING USING SCREWDRIVER. SCREWDRIVER SLIPPED CUTTING RIGHT INDEX FINGER.

SUPERVISOR AND CREW WERE BLDG A WALL AT THE MILL OPERATION. AT APPROX 1115 THE DAY OF 6-22-89 IT WAS TIME TO BACKFILL THE WALL. EE STOOD ON TOP OF THE WALL DIRECTING THE LOADER IN WHO WAS CARR YING THE BACKFILL. THE LOADER OPERATOR STARTED TO DUMP HIS BUCKET LOAD AND A LARGE ROCK ROLLED OUT OF THE BUCKET AND HIT THE TOP TIE HE WAS STANDING ON. HE FELL BETWEEN THE WALL AND THE EMBAN

THE OPERATOR OF3THE 40 TON END DUMP, WAS LOADED WITH LARGE SHOT ROCK. HE ARRIVED AT THE DUMP SITE, TURNED TO THE RIGHT TO POSITION THETRUCK FOR UNLOADING. UPON ENGAGING THEHYDRAULIC BED LIFT, THE DISPLACEMEN7 OF THE WEIGHT WAS ALTERED, THE FILL UNDER THE TRUCKS LEFT REAR WHEEL FAILED. DUE TO THE GRADE & POSITION OF THE TRUCK, IT FELL FROM LEFT TO RIGHT & FROM REAR TO FRONT.

THE EMP WAS WOR5ING THE SECOND SHIFT AT THE PLANT. HE STARTED TO TIGHTEN BLETS ON THE PORTABLE CRUSHER MOTOR. AS IN DOING SO HIS WRENCH SLIPPED AND STRUCK HIM ON THE LEFT SIDE OF HIS FACE
