While removing parts from a cone crusher, a pry bar was being used to spin a part, the bar slipped causing the employee to slip landing on their left knee causing injury.

Employee was using a battery jump pack to start the loader. When switching from 12 to 24 volts the jump pack exploded. The explosion caused parts to fly off the jump pack striking the employee in the groin area . There was also a loud blast causing ringing in both ears.

Employee was performing preshift inspection on the secondary plant screen deck. Employee used left hand and arm to raise a rubber curtain on the feed end of the polydeck. At this time, the employee felt a sharp pain in the left shoulder and down to the left elbow.

The entire plant area was covered in a skiff of snow, with some small icy areas. Employee was walking from secondary plant area to primary area into an area that had not been cleared due to limited access. He slipped on a patch of ice injuring his left wrist.

WHEN THE LAST LITTLE BIT OF STONE CAME OUT 2 ROCKS HIT EACH OTHER AND ONE OF THEM FLEW OVER & H IT ME ON THE LEFT SIDE OF MY FACE. IT WAS A 10" ROCK. IT BROKE MY SAFETY GLASSES AND CUT ME JUST BELOW MY EYE.
