Struck against a moving object

While loading oversize rock into the haul truck a large rock jolted the truck causing the truck operator to sway side to side causing the operator to hit their head on the side door window. The loader operator saw the truck shake but didn`t know the truck operator hit their head.

Over-exertion in pulling or pushing objects

EE was pushing a rollable chute closed and pulled a muscle in their back. The chute track was clear of any obstacles. The EE went to Medic Express for the pain. They gave EE a prescription for pain and placed EE on restricted duty until EE's next visit on 2/22/24.

Caught in, under or between running or meshing objects

Employee was cutting a 4"" black iron steel pipe with a portable band saw. The saw grabbed the pipe and jumped up and the employee's hand went under the spinning blade cutting employee's fingertip off just behind the fingernail.

Struck against stationary object

While stepping out of the tool van the employee stepped on a #4 size stone rolling EE's left ankle resulting in a sprained ankle.

Removing the rock breaker from above the primary crusher. while hooked to the breaker with the crane the employee cut off the last bolt holding the breaker in place. once cut the breaker shifted backwards pinning the employees legs against the hand rail around the platform the breaker was bolted to.

Three employees were pushing a frozen piece of material off of a conveyor. One employee was using a bar the other two were supporting the frozen piece while baring it off the conveyor. Once the piece was off the conveyor one of the employees supporting the frozen pieced noticed his arm was hurting.

Employee was struck by a reducer/motor assembly he was helping install when it moved and hit him on top of the left shoulder.

Employee tried to turn the belt clamp attached to the conveyor belt to be able to get at the bolts holding clamp. When employee tried turning the clamp up, employee felt a pull in his back.

Employee was climbing into water truck cab, stepping up with left leg and right foot on ground, heard a pop & felt a pain in right knee. Upon moving water truck to fill station, when employee got out of truck, then realized could not put any weight on right knee.

Installing rubber disc around spray bars on screen deck. Had to trim disc. Holding disc in place with right hand on the top scribbing line with knife in left hand resulting in a laceration to pointer finger on right hand. The laceration resulted in six stitches.
