Injured person was lowering a feed gate into position with a chain come-a-long. Feed gate came to rest on material and the chain came slack. He then tried to reposition chain when the gate shifted and pinched his fingers. Injury was two broken fingers.

Worker was tightening bolts with a ratchet wrench. The wrench slipped or didn't grab the gear inside of wrench. The handle of the wrench hit the worker in the forehead causing an impact cut.

EE was trying to free up a sand auger. He had another employee jog the power. He reached in the v-belt area just as power was turned on - caught thumb & finger in V-belt.
