Suncore Construction and Materials Inc. · Struck by falling object

Employee was assembling legs for conveyor belt on portable crusher. Leg fell out of EE's hands and struck EE's left foot and ankle. Contusion to left foot.

Miner was changing a v-belt on a cone screen. He was rolling the belt on the pulley and pinched his thumb between the belt and pulley. He had placed his thumb in the wrong location. As a result he received a laceration to his thumb. During the investigation it was found that LO/TO was being used as well as necessary PPE.

Miner trying to free up rock stuck in machinery. Standing on material, material shifted when rock dislodged, his ankle smashed between couple of rocks. Reason for late filing, conflicting stories reported by miner during investigation. Not hospitalized-observation & time off. Before date of inj EE scheduled day off 6/25, 1 holiday 6/28.

EMPLOYEE WAS WORKING ON AGGREGATE CONVEYOR BELT. WHEN HE JUMPED OFF BELT, AIR GUN HOSE GOT CAUGHT ON RAIL. AIR GUN HIT HIM ON HIS HEAD AFTER HE JUMPED FROM BELT.
