January 12, 2022 UT · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer SLIP OR FALL OF PERSON

Employee was performing a pre-trip inspection on EE's trackhoe, as EE was stepping down EE missed the track and fell backwards off the equipment landing on a concrete pad. Struck EE's head and back. Non-displaced fracture at L2-L4 right transverse process. Employee was taken off work at this time, estimated four weeks.

The injured Miner was cutting a broken piece of conveyor belt material that had been tangled into a tail pulley. In doing so, the miner cut his left index finger with a utility knife.
