Victim was removing high voltage disconnects when he came in contact with high voltage power line. He was transported to Logan Medical Center, air lifted to Cabell Huntington Hospital. Left Arm AMPUTATED below shoulder & Right Leg AMPUTATED above knee. Not Chargeable per Logan WV field office. Mine was abandoned 7/25/05.

Struck by rolling or sliding object

Folding MRS cable along rib, rib rolled off and knocked employee to the ground. Injury to left shoulder, neck and side of head.

WHILE HELPING ELECTRICIAN TAPE TRAILING CABLE ON SHUTTLE CAR HE FELT A SHOCK IN HIS RIGHT HAND. INJURED EMPLOYEE CONTINUED WORKING WITHOUT INCIDENT UNTIL THE MORNING OF 10/4/04.

Tripped and fell while getting on mantrip.

Injured slipped and fell due to soupy slick mud and went around head drive. Bottom was uneven.

Putting guard on shuttle car and it slid off striking right foot at the toes.
