Employee was removing bolts on the cutting edge of the loader and sliced his left index finger. He received 7 stitches. He has returned to work today.

EE WAS WALKING ACROSS A PIPE ON THE SCREENING PLANT WHEN HE LOST HIS FOOTING & FELL. BROKE HISRIGHT RING FINGER TRYING TO BREAK THE FALL. EE HAD NO LOST TIME AT TIME OF INJURY. LATER ON FING ER STARTED TO BOTHER HIM WENT OUT ON WC 1/23/01 HAD SURGERY

PLANT WORKERS WERE HOISTING A GEAR REDUCER TO AN ELEVATED WORK PLATFORM BY MEANS OF A POLYPROPELENE ROPE THAT RAN FROM THE REDUCER THROUGH A PULLEY AND BACK TO A LOADER. AS THE REDUCER NEARED THE PLATFORM, THE PULLEY CAME LOOSE FROM ITS' POSITION (ABOVE AND BEHIND THE WORKERS). THE HOISTING ROPE CROSSED THE UPPER BODY OF I.NAME, FORCING HIM OVER THE HANDRAIL.

A PULLEY BEING USED TO HOIST A GEAR REDUCLER TO AN ELEVATED WORK PLATFORM CAME LOOSE. THE HOSIT ROPE FORCED ONE WORKER OVER A HANDRAIL DETAILED IN SEPARATE REPORT AND APPARENTLY KNOCKED EE INT O A NEARBY HANDRAIL AND LADDER.
