Two employees were removing and reinstalling a tail pulley bearing on a conveyor. There was no triggering incident or acute pain/injury, but one of the employees felt increasing pain in their left shoulder later that night and reported it to their Site Manager the next morning.

WAS RUNNING LOADER. WHEN FINISHED PARKED MACHINE. EXITED MACHINE AND PROCEEDED DOWN STEPS/LADDER USING THREE POINTS OF CONTACT. ONCE ON THE GROUND, TURNED TO WALK TOWARDS THE PLANT - SLIPPED ON ICE AND FELL TO THE GROUND. LANDED ON BUTT / LEFT SIDE.

Over the road trucking contract driver had been successfully loaded. Made his way to designated tarping area. While engaging manual tarping mechanism the handle malfunctioned causing truck driver to lose balance from bottom rung of ladder. Driver slipped and struck parts of his truck. Suffered lacerations to "rear end and hand". lacerations required sutures(stitches).
