At our annual hearing test we had an employee that had a significant shift in hearing. I received an e-mail from, Aggregate Industries Safety Manager, that I needed to file a 7000-1 report.

Amrize Midwest Inc. · Caught in, under or between a moving and a stationary object

EE pinched his right middle finger between the fork on fork truck and some steel. Nail turned color but did not want medical attention. Finger continued to give him problems. On 8/18/12 went to clinic found infection and broken finger.
