Pioneer Sand Co Inc · Struck against a moving object

Individual operating scraper, traveling empty along mine floor, moved off track to allow truck traffic. Hit rough area causing EE to hit head against right and left side of cab, leaving EE unconscious. EE believes EE began to can down before hitting head, which stopped the scraper shortly after EE lost consciousness.

Was breaking a bolt loose when the wrench slipped causing hand to hit conveyor frame resulting in laceration to middle knuckle and pinky finger.
