Ee was installing a conveyor leg brace. He had aligned the top hole and inserted a bolt but no nut. He was aligning the bottom to install a bolt when the top slipped off and fell on his leg. (1 missed day & restricted duty.)

A BRIDGE FORMED IN THE FEED HOPPER, THE MINER SHUT DOWN THE CONVEYER AND CLIMBED ONTO THE CONVEYER TO CLEAR THE BRIDGED MATERIAL. AS HE WORKED HE STEPPED ONTO A ROLLER WHICH ROLLED OUT FROM UN DER HIS FOOT AND HE SLIPPED DOWN INTO THE CONVEYER FRAME, TWISTING HIS ANKLE IN THE PROCESS.
