EE came to work on 3/30/09 and told his supervisor he was hurt. At that time, he was told to go to the doctor. He did not report any injury to Ohil Stone, Inc on 3/26/09. His Dr report says that was the injury date.

Was tearing 3' crusher apart, had to remove overflow chute. When pulling on it to dislodge the chute, rocked back onto little finger, cutting finger.

SLIN G SLIPPED OFF OF SHAFT & LANDED ON HIS LEFT FOOT. SAID HE WAS FINE & DID NOT NEED A DOCTOR. BUT WENT LATER TO ER FOR XRAYS. NOTHING WAS BROKE.
WAS PUTTING NEW SCREENS ON DECK, SLIDING SCREEN DOWN INTO THE DECK HAND HIT TOP DECK SHARP EDGE WHERE STONE FALLS THRU SCREEN DECK, CUT RIGHT MIDDLE FINGER WAS WEARING COTTON GLOVES WENT TO EMERGENCY ROOM GOT 8 STITCHES WENT BACK TO WORK

EMPLOYEE WENT TO CLIMB UP IN BODY SECTION OF RRK #187, HE PLACED HIS FOOT IN THE STEP & THE OTHER FOOT ON TRK BODY FLOOR-WHEN HE WENT TO FULLY STAND UP HIS FOOT SLIPPED & HE FELL LANDING ON LE FT LOWER LEG. THERE IMMEDIATELY APPEARED A LARGE KNOT UNDER SKIN. THE KNOT WAS APPROX SIZE OF ORANGE. HAD X-RAY & RETURNED TO WORK.
THEY WERE PICKING UP A MOTOR WEIGHING BETWEEN 450-500 POUNDS. DID NOT REALIZE MOTOR WAS THAT HEAVY. WILL USE MORE CAUTION. LOWER BACK STRAIN RESULTED.
