November 11, 2010 OH · Metal/Non-Metal FALL OF ROOF OR BACK
Sidwell Materials Inc. · Accident type, without injuries

Roof fall appears to have originated at or near the face in heading 1 & extended approx 100 to 120' outby. It is approx 30-35' in width at its widest point & extends above the roof line approx 15-19'. It is believed that a combination of low cover, lower structural quality of roof rock & possible localized horizontal stress had created the conditions leading to the failure.
