Employee was trying to straighten a roof bolt that was slightly bent using the step on the Fletcher Roof Bolter when the bolt rotated and slipped while in the step causing the employee to hit his left wrist on the step causing it to have a small slight fracture to his wrist.

Roof fall appears to have originated at or near the face in heading 1 & extended approx 100 to 120' outby. It is approx 30-35' in width at its widest point & extends above the roof line approx 15-19'. It is believed that a combination of low cover, lower structural quality of roof rock & possible localized horizontal stress had created the conditions leading to the failure.
