EMPLOYEE WAS CL9ANING VIBRATOR ON WASH PLANT AND IN PROCESS OF CLEANING, HE WAS MOVING A SHAFT WITH A BERING HOUSING ON. THIS HOUSING HAD BEEN LOOSENED THE PREVIOUS DAY, BU TEMPLOYEE WAS UNAW ARE OF THIS. T2E BEARING HOUSING CAME OFF SHAFT AND FELL ON HIS RIGHT ARM.
EMPLOYEE WAS HE5PING WITH REPAIR OF GRAVEL SCREENS. HE WAS STANDING NEAR WELDING MACHINE WITHOUT ANY PROTECTIVE DEVICES ON HIS EYES AND THE GLARE FROM WELDER BURNED HIS EYES.
EMPLOYEE WAS RE1AIRING CONE CRUSHER USING A HAMMER TO REMOVE SOME PART FROM IT. HIS FOOT SLIPPED AND CAME IN CONTACT WITH HAMMER.
