AFTER FUELING EQUIPMENT, THE EMPLOYEE WAS ENTERING MACHINE AND HIS FOOT SLIPPED OFF THE LADDER CAUSING HIM TO FALL AGAINST LADDER. EMPLOYEE STRUCK HIS CHEST ON THE LADDER RESULTING IN A FRACTU RED RIB ON HIS LEFT SIDE. MACHINE WAS PARKED AND SHUT DOWN FOR FUELING WHEN THE ACCIDENT OCCURRED.
