HE WAS NOT SCHEDULED TO WORK 6-22-91 AND WENT TO TWIN CITY HOSPITAL WHERE HE WAS TREATED & RELEASED. INJURY WAS NOT REPORTED TO HIM UNTIL 7-12-91.

AT THIS TIME THE OPERATOR REVERSED THE CARRIAGE SO THE NEXT AUGER FLIGHT COULD BE SET IN HE WAS CAUGHT BETWEEN THE MAIN ENGINE FUEL TANK AND REAR LIFTING JACK SUSTAINING TRAUMA TO HIS BACK AND RIGHT HIP.
