Irving Materials Inc · Struck by flying object

EE started having eye irritation on 10/11 at 0900 hours. It is believed that a shard of metal from grinding the previous day caused the irritation to the eye. The flake of metal was removed and EE given antibiotics for this injury. EE was returned to work with no restrictions. During the grinding process EE was in full and appropriate PPE.

WHILE SHOVELING SAND TO MAKE SMALL TRENCH HE TWISTED AROUND TO DUMP SHOVEL WHEN BACK MADE POPPING SOUND LIKE BREAKING A STICK. LOWER LEFT SIDE OF BACK WAS VERY PAINFUL BUT COMPLETED WORK DAY & WENT HOME.DECIDED LATER THAT NITE TO GO TO E.R. WHERE DR. EXAMINED & PRESCRIBED MEDS WITH LIGHT DUTY. WAS TOLD IF PAIN PERSISTS TO SEEK FURTHER MEDICAL ATTENTION.

WHILE PICKING MUD & ROCKS OFF OF CONVEYOR BELT, HE REACHED OUT TO PUSH A ROCK OFF THAT WAS HEAVIER THAN EXPECTED. THIS SPRAINED HIS RIGHT SHOULDER & HE NOTIFIED FELLOW EMPLOYEES & MANAGEMENT IMMEDIATELY. HE CHOSE NOT TO SEEK ANY MEDICAL ATTENTION UNTIL 07-12-2013 AFTER CONTINUED SORENESS PERSISTED.

EE was doing maint. on crane his foot slipped off of step. The EE fell backwards striking tracks with leftside before landing on ground in a sitting position. Dr. said to take it easy over weekend and gave him medication for first few days. RTW Monday with no loss time or restrictions.

While attempting to set a cable into the eye of a dragline bucket, he pulled back hard on the cable. This caused him to raise up & strike his hard hat on the pulley jamming his neck & back. He did not seek treatment until that evening. He was prescribed medication. After taking medication he had a severe allergic reaction causing a hospital stay & time off with restrictions.
