Injured was checking fluids on EE's security truck. EE slipped/tripped on the ground and landed on EE's arm.

Injured was welding in the surface shop. A piece of steel fell and hit EE's right foot. An MRI was performed on 7-17-2020 and EE was placed on restricted duty at that time for a Non-Displaced Fracture in the foot.

Injured was changing out a basket in the prep-plant. As he was cleaning the old basket a piece of dirt got in his eye. He went to the Dr on 4-13-15 and his eye was flushed out with prescription medication.

Injured was lifting a piece of floor grating and cut his finger. The laceration required four stitches.
