Miner was assisting in changing out a screen. The screen slipped while being moved into place and struck the miner's hard hat. The hard hat slipped over EE's face and impacted EE's nose, breaking it.

EE WAS CHANGING ROLLERS ON CONVEYOR. CONVEYOR WAS SHUT OFF BUT NOT LOCKED OUT BY EES. ANOTHER EE FELL AGAINST CONTROL PANEL STARTING CONVEYOR. EE WAS LAYING IN RETURN PORTION OF CONVEYOR AND L EG WAS DRAWN INTO PULLEY.
