Employee cleaning screen top deck chute and employee was using a long pry bar to remove material buildup when the bar slipped through his right hand pushing his hand into the chute. Causing a contusion to his right hand.

Employee was feeding crusher when going up ramp dropped a rock. When going down hit rock, jolting cab. Didn't feel stiffness until after work. Reported next morning to Leadman (10-06-04). Did not request medical attention from Company until 10-7-2004.

EMPLOYEE WAS INJURED IN A MOTOR VEHICLE ACCIDENT WHEN A 988F LOADER BACKED OFF THE WASH PLANT FEEDER RAMP AND STRUCK THE WATER TRUCK IN WHICH HE WAS SEATED. THE SPEED OF THE LOADER AT THE TIME OF THE COLLISION WAS WELL BELOW 5 MPH AND THE WATER TRUCK WAS STOPPED. THE EMPLOYEE WAS WEARING A SAFETY RESTRAINT AND HARD HAT.
