While employee was looking at project at tailpulley of Stacker belt, bungy cord holding S-O cord broke. Bungy came down and hit him on top of head.
Employee and a coworker were splicing a conveyor belt at the washplant. When the belt clamp was removed it fell on employee's right index finger
While employee was changing screens on washplant, his safety harness caught on spray bar. He slipped and strained his back.
Employee was driving haul truck when he hit a pocket of silt which jolted the truck. He experienced pain in lower back.

Employee was feeding crusher when going up ramp dropped a rock. When going down hit rock, jolting cab. Didn't feel stiffness until after work. Reported next morning to Leadman (10-06-04). Did not request medical attention from Company until 10-7-2004.

EMPLOYEE WAS INJURED IN A MOTOR VEHICLE ACCIDENT WHEN A 988F LOADER BACKED OFF THE WASH PLANT FEEDER RAMP AND STRUCK THE WATER TRUCK IN WHICH HE WAS SEATED. THE SPEED OF THE LOADER AT THE TIME OF THE COLLISION WAS WELL BELOW 5 MPH AND THE WATER TRUCK WAS STOPPED. THE EMPLOYEE WAS WEARING A SAFETY RESTRAINT AND HARD HAT.
