Was cutting belting with utility knife. Knife slipped and cut thumb on left hand.
he was preparing to do a stone sample. He picked up two buckets of 2B Blend stone that was approximately 44 to 50 lbs. Hurt his back.
employee worked 7PM to 7AM shift. Before dark his bright vest was covered with little black bugs. As the night progressed, he started getting bites on him. By morning he had several bites, a welt on his arm, and his lip was swollen up.

Walking outside along the shop building to unlock diesel fuel tank. Slipped and fell due to snow and icy conditions. Hurt his left ankle.

He was lifting buckets of stone onto the bed of a pick-up truck. Felt a pop in his right shoulder.

Exiting outside lab area and slipped on threshold of door. Tripped through doorway and caught himself on side of his truck which was parked outside of the door. He felt a pop in his left hip.
An impoundment failed allowing water to flow eventually into a nearby creek. There were no injuries or property damage.The water flowed into a low lying area first and then into the main settling ponds and then into a creek. There had been an great deal of rain in the past two weeks.

Employee was using a drill to make bolt holes to mount an air conditioner unit. While drilling into the metal, a sliver of metal went into his right eye.

He was on his knees cutting tail wiper. He stood up and started walking, and a sharp pain flew in right hip.

He was replacing ceramic tiles, breaking them away with a chipping hammer, a tile splintered and a piece went into his middle finger on his left hand.
He was taking the crusher apart using a hammer and wedges. A piece of the wedge broke off and stuck in his cheek.
He was gathering a sand sample. He inserted sample tube into the the sample pad of sand, and it stopped abruptly, striking him very hard on his right forearm.
Cutting belting, caught finger on belt clamp that was sticking up on belt. Laceration on left hand, middle finger.
Using utility knife to cut belt, slipped and cut left thumb.
He just bent over to pick up a bucket of sand in the lab, and hurt his back.
