107 & 106 conveyors had overloaded & tripped out. Therefore, choked up the chute that they were working on. The employee used a bar with a hook on it to move the clay around. In the process of getting the system back on line he hurt his right shoulder.

A PIECE OF SHEET METAL GOT CAUGHT UNDER THE 108/102 TRANSFER POINT. HE WAS PULLING THE METAL SHEET OUT AND IT CAUGHT ON THE TRANSFER POINT HIS HAND SLIPPED AND THE SHEET METAL CUT THROUGH HIS GLOVES.

HE WENT TO MOVE AND SLIPPED, HE PUSHED HIMSELF AWAY FROMT HE TANK AND LANDED ONT HE GROUND 3.5 FEET BELOW. HE LANDED ON HIS ANKLE. HE STRETCHED A TENDON IN THE TOP OF HIS FOOT. X RAY WAS NEGATIVE.
